I see it more as a hotel or apartments. It really is a nice building. If the listing is correct, it only has just over 100 parking spaces. That would make any large commercial use a bit of a challenge. (I wouldn't see it as a challenge, but I'm not in "pry my car from my cold dead hands" camp.)

Check out the floorplans in the marketing pdf. Looks perfect for hotel or small apartments.
The lot is about the exact same size as 400 S Jennings if a developer wanted apartments + garage, or the same size as the Magnolia Green townhomes and Alston garage if a developer wanted some condos or town homes + parking on the site as an addition.
Also the little green patch along Lancaster would be great for dogwalking.
Dang, really want to live here now!

I didn't even scroll down to see the rest of listing. It really has a lot of potential. I didn't realize the full size of the lot. I know I've been in the building before, but it would have been prior to 1989 and I've forgotten far more than I remember since then.

First and foremost, my vote would be for a Hotel with Spa; it certainly has the bones (hospital rooms, cafeteria, etc.)

Is the COFW HCLC required to approve a National Register of Historic Places nomination? I was under the impression that the COFW HCLC had more strict guidelines than the National Register. What step in the process below is this currently, #2?

I don't think I have answered this question before, and I'm not sure I have all of the answers. The COFW requirements are tougher than the National Register's for individual nomination. From what I can remember, I believe that it is our City's Preservation Ordinance that the HCLC reviews and approves the National Register's nomination of a structure within our city limits. This approval is only a recommendation.
The notification was sent yesterday that the old W.I. Cook Children's Hospital on Lancaster is now officially listed in the National Register of Historic Places.
